Job Summary:

The Monitoring and Evaluation Officer will be responsible for tracking project progress, monitoring patient outcomes, and conducting follow-ups with patients, especially those classified as mid- and high-risk. The role ensures that the project goals are being met and that patients are receiving the necessary care.


Key Responsibilities:

  • Track the progress of health initiatives and ensure project milestones are met.
  • Monitor and evaluate patient outcomes, particularly for mid- and high-risk patients.
  • Conduct regular follow-ups with patients to assess their recovery and care.
  • Collect and analyze data to provide insights and feedback to the project team.
  • Prepare reports on monitoring and evaluation findings for district-level management.

Requirements:

  • Master’s degree (any discipline).
  • Strong analytical and data management skills.
  • Experience in monitoring and evaluation processes preferred.
  • Ability to work with healthcare teams and follow up with patients.
  • Good communication and reporting abilities.
