Job Summary:
The Monitoring and Evaluation Officer will be responsible for tracking project progress, monitoring patient outcomes, and conducting follow-ups with patients, especially those classified as mid- and high-risk. The role ensures that the project goals are being met and that patients are receiving the necessary care.
Key Responsibilities:
- Track the progress of health initiatives and ensure project milestones are met.
- Monitor and evaluate patient outcomes, particularly for mid- and high-risk patients.
- Conduct regular follow-ups with patients to assess their recovery and care.
- Collect and analyze data to provide insights and feedback to the project team.
- Prepare reports on monitoring and evaluation findings for district-level management.
Requirements:
- Master’s degree (any discipline).
- Strong analytical and data management skills.
- Experience in monitoring and evaluation processes preferred.
- Ability to work with healthcare teams and follow up with patients.
- Good communication and reporting abilities.
